The identification of a biomarker for myopia development is an appealing prospect, considering that its incidence is increasing worldwirde, and that high myopia can severely affect visual health. This article will explore the role of choroidal changes induced by defocus and their implications for myopia development. Hyperopic defocus seems to promote choroidal thinning, and this could weaken the structure and facilitate axial elongation, which is a key feature of myopia. Moreover, the greatest choroidal thinning is detected in children newly developing myopia, suggesting that early infancy is the most crucial period for myopia interventions. However, more longitudinal studies with larger ad more homogeneous samples are needed to understand this mechanism.
